B.        QUALIFICATIONS

 

Knowledge of agency policies and procedures.  Knowledge of diagnostic impressions or criteria.  Knowledge of how client advocacy systems operate.  Knowledge of crisis intervention techniques. Ability to work and communicate effectively with diverse populations. Entry qualifications include a Master's degree and eligibility to be licensed Or Bachelor's degree in a related field and two years of experience in social service delivery.

 

Possess basic reading, writing, and math skills; able to lift 50 pounds; able to complete Crisis Intervention training; able to use a computer; able to maintain a valid driver's license and able maintain valid auto insurance.

Work requires the knowledge and skills necessary to provide direct consumer services appropriate to Developmental Disabilities whose ages range from 18 to retirement.

                    

Staff should be able to:

 

a. Demonstrate knowledge and skill in non-violent crisis intervention techniques.

b. Display knowledge of agency, community, and institutional resources and referral processes.

c. Assist people supported and staff in appropriate goal setting and implementation of the treatment plan; display knowledge of discharge planning

d. Display knowledge of HFR, Regional Board, CARF, and AACSB compliance requirements.

 

H.        ASSIGNED DUTIES

 

DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES:  The agency expects employees to perform their work in a competent and efficient manner.  Working as part of a team, under limited supervision, the Lead Day Care Plan Coordinator will be responsible for ensuring services delivered at Primus are delivered through Person-Centered Practices and provided in accordance with agency policies and procedures. Will assist with training I/DD direct support professionals on the proper implementation of plans and ensuring the plans meet the physical, social and developmental needs of each person supported. The plan should also protect the person's health, dignity, safety, comfort, well-being, civil, human and legal rights.

The primary work location is Primus I/DD Day Service Center; however, per need of the agency, work may occur in another Center, the residence or community. 

 

The Lead CPC provides direct supervision to assigned direct support professionals and supports other managers in the day-to-day oversight of instructional units.

 

INDIVIDUAL SUPPORT:

  • Act as liaison, advocate and coordinator for the people supported in involvement of support groups, self-advocacy, vocational programming, transportation services, integrated community activities, recreation and learning independent living skills.
  • Observation of instructional area at least twice (2x) monthly for each person on assigned caseload; documentation will be reflected in DDP reviews.
  • Foster open and supportive communication with people served, families and agency stakeholders and other community agencies.
  • Oversee the medical, medication and health issues of people supported on assigned caseload.
  • Provide and ensure communication of medical and health issues to staff, agency nurses, families, etc. using appropriate logs when necessary.
  • Implement and follow-up on all support services recommendations.
  • Develop and Implement Community Access Individual (CAI) Calendars based on Individual Support Plan.
  • Develop Community Integration Calendar based on feedback from FOCUS Planning and Self-Advocacy Representative Feedback.
  • May be required to cover a classroom when required by staffing ratios. 

 

SUPERVISON/TRAINING:

  • Monitor staff performance management, including evaluations and developmental plans, disciplinary actions for all assigned direct support professionals as scheduled; not limited to but a minimum of quarterly supervisory conferences and annual performance reviews.
  • Serve as Developmental Disabilities Professional (DDP) by providing observation and oversight of direct care professionals who are providing instructional support services at Primus Day Services.
  • Provide guidance and support to instructors on the development and implementation of activities (curriculum).
  • Provide guidance and support to direct support staff who are  unable to manage challenging and/or inappropriate behaviors. 
  • Assist with monthly trainings for staff based on Person-Centered Thinking, QLARANT(ASO) Focus Areas through teaching, acting as a role model and guiding staff on how to appropriately work with and interact with people served, other staff, families and guardians.
  • Work closely with agency Care Plan Coordinators to monitor day service documentation for compliance including but not limited to : goal documentation, behavior tracking, progress notes, community outing logs, medication administration records.
  • Present and implement agency policies and procedures.
  • Participate in the interviewing and hiring of new staff.

  • Maintain open communication with supervisor of pertinent information including but not limited to staff, scheduling, and budgeting, incident reporting.
  • Assist with CSDC duties in absence of supervisor.

 

ACCOUNTABILITY:

  • Review and approve progress notes daily via CareLogic (EMR) based on person-centered documentation, accuracy and correct frequency.
  • Monitors Prior Authorizations for Primus Day Services to ensure timely submission.
  • Monitor Failed Activities for Primus Day Services.
  • Monitor 5% Audit Corrective Action Plans for Primus Day Services.
  • Complete, Submit and Monitor Exceptional Rate Requests under supervision of Behavior Specialist.
  • Assist with collection of Behavior Support documentation and ensure direct support professionals are implementing services as outlined in plan, tracking behavior logs, coordinating with Aspire Behavior Specialist and assisting with training direct care professionals on Behavior Plans.
  • Ensure completion/submission of all required daily, weekly, monthly, quarterly and annual reports as scheduled.
  • Routinely review all required reports, logs, mail, requests, etc. as needed, maintaining an organized filing system.

OTHER:

  • Works collaboratively with other departments to communicate medical, social and behavioral needs of individuals served in a timely manner, as well as, the daily operational needs of the program. Represent I/DD at approved community activities, health fairs and career fairs to promote I/DD Services
  • Develop pertinent educational and trending I/DD topics for communication boards,as well as, monthly topics for Rights, Self-Preservation, Safety and Advocacy.
  • Demonstrate and foster the characteristics of Integrity, Respect, Teamwork, Professionalism, Accountability and Innovation.
  • Perform additional duties or special projects as required by the nature of the position or as requested by supervisor.
  • May be required to transport consumers using your personal vehicle or an agency vehicle.
  • Duties as assigned.

 

Normal hours: Monday through Friday, 8:00 a.m.- 5:00 p.m. However, flexibility is required to work various day, evening, night and weekend shifts when needed. May be necessary to participate in emergency call rotation or cover shift if adequate coverage cannot be obtained by direct care staff.
