Minimum Training and Experience: Master’s degree in social work, psychology, counseling, sociology, education, nursing, vocational rehabilitation or a related field OR related field and four (4) years of full-time (or equivalent part-time) work experience in social services delivery (case management, outreach, prevention/education, etc.).  This position requires a license (LPC, LMFT, or LCSW).


Physical: No lifting of more than 20 pounds. May transport consumers using agency vehicle only (AAA Certification required). 


Incumbent may possess the following clinical/organizational skills: Supervision of Clinical staff in a multidiscipline team approach; experience in working with external review organization; pre-authorization experience for service provision, high productivity in the workplace; successful (documented) management experience in the behavioral health field, agency-wide problem solving with corrective action in place (documented); serving on regional or statewide workgroups or task forces; any product that demonstrates a high level of organizational ability working across disability sections with a variety of managers; effective writing and communication skills

  1. Age Related and Special needs of population served:

Work or Volunteer experience with individuals with mental illness, and alcohol/drug addiction.  Knowledge and skills necessary to provide direct consumer services appropriate to Adults having Mental Illness, and or Addictive Disease disorders whose ages range from age 18 to older adults. Complete essential learning and maintain related annual training requirements.

  8. Knowledge of Psychosocial Rehabilitation and Peer Support services.


  1. Assigned Duties.
  2. Staff will provide a broad range of services, including Individual, Group, and Community Support to clients with chronic mental illness and/or addictive diseases in clinic, and or community settings.

    Assist clients in accomplishing goals and maintaining stability through using evidence based therapeutic interventions and by providing psychoeducatioal groups or teaching daily living skills, applying for supportive services and assistance, monitoring health, financial management, provide transportation and appointment setting.  Create person centered treatment and discharge plans. Documents progress toward goals and objectives in each client's electronic medical record.

    Works and builds relationships with other service providers and staff in support of effective client treatment, coordinating, communication and continuity of service. Keeps supervisor and other treatment team members informed of significant developments.

    Percentage of Assigned Duties

70%           Deliver billable clinical services (BHA, SPD, Group or Individual counseling, crisis intervention, or paraprofessional services they may be required.  Maintain 70% productivity.


25%           ensure case loads are accurate and services are authorized. Ensure documentation is current and within 24 hours. Ensure OTRs are approved and corrections and follow up are made to obtain authorizations on Connects or OTRs. 


4%             participate in unit specific meetings and training.


1%             Other duties as assigned, not to exceed 90 days, as assigned by supervisor. Will keep up with essential learning to perform assigned work.
